There’s so much to protect around you, and most importantly your loved ones. Each situation is unique and that’s why the Blue Springs School District offers a variety of affordable term life insurance options through Forest T. Jones to help plan for some of life’s unexpected situations.

Basic Employee Life Insurance

We provide you with $15,000 of term life insurance at no charge. If eligible, you have the option to purchase additional protection for your, your eligible spouse, and dependent child(ren) up to age 25.

Basic Dependent Life Insurance

Eligible dependents may be added to your Basic Life policy for $0.90 per month. Coverage amounts: Spouse, $2,000; Dependent Children between 25 years and 6 months, $1000; Dependent Children between 6 months and 14 days, $100.

Supplemental Life Insurance

You have the opportunity to enroll in additional term life insurance in increments of $10,000 for you, your spouse, and eligible dependent child(ren).

As a new hire, if you enroll for supplemental employee coverage in excess of $100,000, you are required to obtain approval through an Evidence of Insurability form from Forest T. Jones before coverages are enforceable.

If you forgo coverage as a new hire or want to increase coverages at a later date, you must obtain approval through an Evidence of Insurable form from Forest T. Jones before coverages are enforceable.

The Evidence of Insurability form is available upon request.
