Missouri Public Schools are required by state law to have complete immunization records on all school-age children. Required immunizations must be administered according to the current ACIP (Advisory Committee on Immunization Practices) which include specific spacing between immunizations. Your student may not attend school until immunizations are compliant with Missouri State law.

To schedule an appointment to receive required vaccinations, contact your student’s healthcare provider or access the immunization scheduling page through Jackson County Public Health.

If your child has asthma, severe allergy (anaphylaxis), diabetes, epilepsy or seizure disorder, adrenal insufficiency, or another condition requiring special health procedures or medications, we ask that you complete and submit the appropriate form(s) to your student’s building or upload to PowerSchool under the “Health” section – all forms must be updated annually.

Forms can be accessed below and printed or copies can be requested from the building’s health services clinic. Forms from your healthcare provider are also acceptable. Contact Health Services staff at your student’s building with any questions or for paper copies.

NOTE: Requests for medication administration and/or medical procedures/treatments will require written orders from a licensed medical provider and permission from the parent/guardian.

Completed forms or orders may be uploaded into PowerSchool on the “Health Form” or submitted to the health clinic at your student’s building.

Will’s law in the state of MO, outlines the training and care plans that schools will implement for students with epilepsy or seizure disorder at the parent or guardian’s request.

We must have written orders from the student’s physician (you may use the Seizure Action Plan) along with a signed authorization to release medical records that allows the provider to communicate and share medical information with us. This enables the school nurse to develop individualized care plans for your student.

If you wish for your student to self-carry and self-administer their emergency rescue medications to treat asthma and/or anaphylaxis (this generally applies to students in secondary grades), you will need to submit a BSSD Asthma & Anaphylaxis Medication Self-Administration Form or equivalent documentation, along with a treatment plan/asthma action plan from the student’s provider. 

Upon written request from a parent/guardian and authorization by a student’s diabetes medical management plan, a student with diabetes will be permitted to carry all necessary supplies and equipment to manage and care for their diabetes while at school or school-related activities. You will need to submit a Blue Springs Diabetes Medication Self-Carry & Self-Administration Form or equivalent documentation, along with a treatment plan from the student’s provider.

Secondary students (grades 6-12) may carry over-the-counter (OTC) medications provided the student carries no more than one day’s supply of the medication in the original container along with written authorization from the parent/legal guardian to self-administer the medication according to package directions. Parent/guardian permission can be handwritten or completed on the Blue Springs School District OTC Medication Self-Carry form.
